Basic Format:

Author Last name, First Name. Title of Book. City of
Publication: Publisher, Date of Publication. Name of Subscription Service. Library from which you accessed the online book, Library's state. Date you accessed the online book <URL of the source or, if the URL is long, the URL of the site's search page>.
